Is there a way to remotely access and listen to audio?

I'm not sure if what I am about to ask will make any sense ........ but please do your best not to think I have gone crazy!

Introduction

I am aware that software like airsonic will allow you to remotely access and stream audio whereever you have a connection to the internet.

My question

I was wondering is it possible to access the logitech media server remotely and have access to my music whilst I am away from home?

I am trying to figure out how this might work, my PiCorePlayer would need to remotely connect to my home sever as would my phone which I use to control LMS.
